Cycle, bleeding & hormones

·
Irregular cycle
Cycle < 21 or > 35 days, spotting, intermenstrual bleeding — hormonal work-up.
→
·
Heavy / prolonged period (menorrhagia)
Changing every 1 – 2 h, clots, fatigue. Causes: fibroids, polyps, hormones, clotting.
→
·
Absent period (amenorrhoea)
More than 3 months without bleeding — pregnancy, hormones, PCOS, early menopause.
→
·
Postmenopausal bleeding
Bleeding after 12+ months without a period should be medically evaluated.
→
·
PMS / PMDD
Mood changes, breast tenderness, water retention before period — counselling & treatment.
→
·
Polycystic ovary syndrome (PCOS)
Cycle irregularity, acne, hirsutism, weight, insulin resistance — diagnostics & lifestyle.
→

Pelvic pain

·
Period pain (dysmenorrhoea)
Cramping pain before / during period. Severe pain: rule out endometriosis.
→
·
Endometriosis
Endometrial-like tissue outside the uterus. Pain, fertility issues — sono, clinic, therapy.
→
·
Adenomyosis
Endometrial tissue in uterine wall. Diagnosis via ultrasound.
→
·
Ovarian cysts
Often harmless; for large, persistent or symptomatic cysts: ultrasound & follow-up.
→
·
Uterine fibroids
Benign muscle nodules of the uterus. With symptoms: sono, medication or surgery.
→
·
Painful intercourse (dyspareunia)
No taboo — causes are usually treatable (dryness, endometriosis, pelvic floor).
→
·
Vulvodynia / chronic pain
Burning, stinging without infection — multimodal approach.
→

Pregnancy & birth

·
First pregnancy consultation
History, clinical examination, counselling and indicated laboratory tests. An early scan before weeks 12–14 is performed only for a clinical question; it is not an additional routine benefit.
→
·
Pregnancy nausea (hyperemesis)
Severe vomiting, weight loss, dehydration — diagnosis, therapy, hospital cooperation.
→
·
High-risk pregnancy
Hypertension, GDM, multiples, prior conditions, prior preterm — close care with perinatal centre.
→
·
NIPT (non-invasive prenatal test)
Maternal blood test for trisomy 21, 18, 13 — neutral counselling.
→
·
Organ screening wks 20 – 22
Detailed ultrasound of fetal anatomy — SGUM-certified.
→
·
Postpartum & breastfeeding
Postpartum visits, pelvic floor, lactation, contraception, postnatal mood.
→

Fertility & family planning

·
Difficulty conceiving
Basic diagnostics (hormones, ovulation, tubal patency), simple therapies — IVF referral if needed.
→
·
Anti-Müllerian hormone (AMH) / ovarian reserve
A marker of quantitative ovarian reserve and expected response to stimulation. AMH does not count eggs and poorly predicts the chance of spontaneous pregnancy.
→
·
Choosing contraception
Pill, IUD, implant, ring, patch, barrier — decision support, no pressure.
→
·
IUD insertion
Copper or hormonal coil, in-office insertion, ultrasound check.
→

Menopause

·
Hot flashes & night sweats
Classic vasomotor symptoms — HRT & alternatives.
→
·
Sleep, mood disturbances
Common perimenopause companions — structured diagnostics.
→
·
Vaginal dryness (urogenital atrophy)
Dryness, burning, painful intercourse — local therapy very effective.
→
·
Early menopause / POI
Menopause before age 40 — diagnostics & specific HRT options.
→
·
Osteoporosis prevention
Bone health postmenopause — vit. D, exercise, possibly bisphosphonates.
→

Breast

·
Palpable breast lump
Triple-assessment: clinical, ultrasound, mammography / biopsy if needed.
→
·
Breast pain (mastodynia)
Cyclic or constant — usually benign; evaluation if persistent.
→
·
Nipple discharge / galactorrhoea
Clear, milky, bloody — colour determines next step.
→
·
Breast ultrasound
Targeted, radiation-free additional imaging — it can complement mammography but does not replace screening mammography.
→
·
Familial breast cancer risk (BRCA)
Risk assessment, genetic counselling where appropriate, and a specialist mammography/MRI plan; ultrasound is an adjunct only.
→

Pelvic floor & bladder

·
Stress incontinence
Urine loss with cough/sneeze/laugh/exercise — physiotherapy & minimally invasive surgery.
→
·
Urge incontinence / overactive bladder
Sudden, hard-to-suppress urge — behaviour, medication, Botox.
→
·
Prolapse
Pressure, “something coming down” — pessary, physiotherapy or surgical assessment through the network.
→

Recurrent UTIs
More than 2 episodes in 6 months — diagnosis & prevention strategy.
→

Vaginal infections (yeast, bacterial)
Itch, discharge, burning — microscopy & targeted therapy.

Sexually transmitted infections (STIs)
Chlamydia, gonorrhoea and trichomonas; Mycoplasma genitalium testing only when indicated. No routine testing or treatment for M. hominis or Ureaplasma.
→
·
High-risk HPV / abnormal Pap
Abnormal cervical cytology or persistent high-risk HPV — colposcopy according to the report and follow-up recommendation.
→
·
Genital herpes
Painful blisters — antiviral therapy, prophylaxis advice.
→

Cancer prevention & routine

·
Cervical screening: Pap or HPV
Ages 21–29: Pap every 3 years. Ages 30–70: Pap or primary HPV every 3 years, not routinely both; HPV screening is not currently covered by basic insurance.

CIN 1 / 2 / 3 — follow-up or conisation depending on grade.
